6/21/2023 EnchantaVaughn Approved Behavior: Jacaeri did not communicate his feelings at the beginning of session and remained quiet while QMHP-C asked about what was on his mind. Once he was able to process his emotions, Jacaeri described that he worries about issues between his parents, among other things. Jacaeri communicated that he didnât feel like talking about his day. Behavior: Jacaeri was given praise and reinforced for participating in the session and developing self-determination skills. Jacaeri continues to display appropriate social skills in therapeutic setting. Intervention: QMHP-C educated Jacaeri on behavior modification to help Jacaeri identify what behavior he needs to decrease and avoid daily. QMHP-C utilized behavior modification system to aid the Jacaeri with following directions given by authority figures. QMHP-C explored the Jacaeriâs capability to cope with his stressors and to get back on track. QMHP-C modeled the correct behaviors and conduct during quiet time. QMHP-C assisted Jacaeri with finding and utilizing necessary resources. QMHP-C verbalized the awareness that all people are unique, learn differently, and have various strengths and weaknesses that can either hold them back or push them forward. Intervention: QMHP-C utilized cognitive skills to give the Jacaeri a chance to openly and honestly discuss his feelings about his current state. QMHP-C utilized active listening skills and was empathetic with the Jacaeri as he vented the overwhelming events that had occurred, resulting in him taking his frustration out on his mother. QMHP-C addressed the Jacaeriâs thinking errors. QMHP-C encouraged Jacaeri to be respectful to authority and peers. QMHP-C reminded Jacaeri to build healthy relationships with his family and use them for support. QMHP-C reiterated the importance of positive thinking and exercising his coping skills. QMHP-C will assist with reducing anger-provoking situations through self-talk and role play scenarios. Response: Jacaeri discussed mental illness and its causes as a family. Jacaeri displayed interest in learning about Attention Deficit Hyperactive Disorder. Jacaeri believes he will get better if he continues to work hard. Jacaeri was acceptive of support and encouragement. Jacaeri exhibited good social behaviors. Jacaeri was able to reevaluate himself and display a more positive attitude toward peers and authority. Jacaeri has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Jacaeri demonstrated expectations and reported a decreased stress level due to his modulation of social contact. Response: Jacaeri responded well to feedback, interacting well with others. Jacaeri implemented his plan for restoring relationships, and this was reviewed. Jacaeri acknowledged QMHP-C and explored his options. Jacaeri proceeded to reduce the aggression in his voice while calming down. Jacaeri accepted suggestions from QMHP-C as evidenced by the Jacaeri smiling and saying thank you. Jacaeri knows what he needs to do to improve socially and academically. Jacaeri expressed to QMHP-C that he understood the importance of intensive in-home and promised to do a better job in each session moving forward.
